DS Clinical Massage – Terms & Conditions
1. Purpose of Treatment
All treatments provided by DS Clinical Massage are intended to support general wellbeing, relaxation, rehabilitation, and functional improvement within the therapist’s scope of practice. Treatments are not a substitute for medical diagnosis or treatment by a GP or consultant.
2. Client Information & Consent
Clients are asked to complete an intake form prior to, or at the start of, treatment. This information is used solely to plan safe and appropriate treatment.
All information provided is treated as strictly confidential
Records form part of the client’s clinical history
Information will only be shared with third parties (e.g. GP, case manager, insurer) with the client’s consent, or where legally required
By proceeding with treatment, clients confirm that the information they provide is accurate to the best of their knowledge and that they consent to treatment.
3. Communication
Clients consent to being contacted regarding appointments and treatment-related matters using the contact details they provide. Messages may be left unless the client specifies otherwise.
Where appropriate and agreed, communication with referring or overseeing healthcare professionals may take place in the client’s best interests.
4. Appointments, Cancellations & Missed Sessions
Appointment times are reserved exclusively for each client.
A minimum of 24 hours’ notice is required for cancellations or rescheduling
Late cancellations or missed appointments may be charged in full
Discretion may be applied in cases of genuine emergency or illness
5. Fees & Payment
Payment is due on the day of treatment unless otherwise agreed in advance.
Where treatment is funded or reimbursed by an insurer, solicitor, or third party:
Responsibility for payment ultimately remains with the client
DS Clinical Massage does not guarantee reimbursement by third parties
6. Professional Standards, Memberships & Safeguarding
DS Clinical Massage is fully insured and works in accordance with professional standards, current best practice, and ongoing continuing professional development.
Professional memberships and registrations include:
Sports Massage Association (SMA)
Complementary & Natural Healthcare Council (CNHC)
The practitioner holds an Enhanced DBS certificate and adheres to appropriate safeguarding principles as part of professional and ethical practice. Documentation can be provided for due-diligence purposes where required.
7. Data Protection
DS Clinical Massage complies with UK data protection legislation. Personal data is stored securely and retained only as long as necessary for clinical and legal purposes.
8. Right to Refuse or Terminate Treatment
The therapist reserves the right to refuse or discontinue treatment if it is deemed unsafe, inappropriate, or outside scope of practice.
9. Acceptance of Terms
By booking and attending an appointment, clients confirm that they have read, understood, and accepted these Terms & Conditions.
